One ballot per membership number.

Sign your name in the blank on the opposite side of the ballot. (You must be the primary member, usually the one who paid the first dues.) Write your membership number in the adjoining blank.

Fold Section A over Section B. Fold Section C so mailing label with signature and member number is on the outside. Be sure the other outside surface is the side with the Co-op's address.
